In July 2024, Bandera was the subject of an investigation by the Occupational Safety and Health Administration (OSHA) following a complaint over a commercial project in Fort Worth.
According to the complaint, workers installing water and sewer pipes lacked enough ladders in the trench and a safe way out in case the trench collapsed.
Bandera Utility has previously been charged for disregarding safety regulations.
When a trench fell during a sewer line repair in 2022, killing a 41-year-old worker, the administration issued a citation for identical safety concerns.
